Who is responsible

Apex Health Hospital, House 1, Road 6, Unity Crescent, Akobo, Ibadan 200222, Oyo State, is the data controller for this website. Under the Nigeria Data Protection Act 2023, a hospital counts as a controller of major importance, which means we carry additional duties for how we handle your information.

What this notice covers

This notice is about the website only — specifically the enquiry form. Medical records held about you as a patient are a separate matter, governed by the same Act and by our clinical record-keeping duties. Ask at the hospital if you want to know about those.

What we collect, and why

If you send an enquiry, we ask for:

• Your name and phone number — so we can call you back. These are the only required fields.
• Your email address — optional, and only used if we cannot reach you by phone.
• The department and a preferred time — optional, so we can route your enquiry to the right people.
• A short description of what it is about — optional. We ask you not to include medical details, test results or a diagnosis here, and we discuss that with you on the phone instead. This is deliberate: keeping health information out of a web form is safer than trying to protect it once it has been sent.

Our lawful basis

Your consent, which you give by ticking the box on the form. You can withdraw it at any time by calling or emailing us, and we will delete what we hold about your enquiry.

Who sees it

Only staff at Apex Health Hospital who need to act on your enquiry. We do not sell your details, we do not share them for marketing, and we do not pass them to anyone outside the hospital except where the law requires it.

How long we keep it

Enquiry details are kept for up to 12 months, and then deleted. If your enquiry leads to you becoming a patient, your care records are kept separately under the hospital’s clinical record-keeping rules.

Cookies and tracking

This website sets no cookies and does not track you. There is no advertising pixel, no social media tracker and no cookie banner, because there is nothing to consent to.

Your rights

Under the Nigeria Data Protection Act 2023 you can ask us to:

• tell you what personal data we hold about you;
• correct anything that is wrong;
• delete it;
• stop using it, where you had given consent.

We will not charge you for any of this.

If you are not satisfied

Please raise it with us first — most things are quicker to fix directly. If we cannot resolve it, you can complain to the Nigeria Data Protection Commission (NDPC), the regulator responsible for enforcing the Act.

Changes to this notice

If we change how we handle your details, we will update this page. It was last reviewed in September 2026.
